For New Members: - Please book the prospective initial visit (Adult/Prenatal/Children) - The initial visit requires a $25 deposit to secure your spot and will be added as credit towards your first visit. We require at least 24 hour notice for appointment changes or cancellations. - The initial visit includes detailed case history, physical/chiropractic examination, postural assessment and your first adjustment if suitable. - Radiate Chiropractic is a cash based practice accepting cash, cards, and checks made out to Radiate Chiropractic LLC. This means we won’t be billing insurance directly, but we’ll happily provide a super bill upon request if you’d like to submit to your insurance for possible reimbursement. We also offer discount opportunities for our service members, first responders, teachers, and nurses. - Once you complete your booking, please check your email to complete your intake paperwork before your visit.
